First Visit

After you have spoken with our office and set up your initial appointment, you will be emailed an invitation to set up your Patient Portal. Please follow the link to create your account, where you will be able to complete all Intake paperwork and sign required forms prior to your appointment. These MUST be completed at least 24 hours prior to your appointment. Please bring your license and insurance card to your appointment. If your appointment is Telehealth, you must upload these to your portal prior to your appointment.

If we do not receive paperwork in advance, the Intake session may be canceled or rescheduled.

Please note that it is our policy to require consent for treatment from both custodial parents in cases of joint legal custody. This consent must be obtained prior to the first visit.

Telehealth

All telehealth sessions will utilize a confidential platform. You will receive a link to a virtual and confidential doxy.me waiting room from your provider or our office with their unique username. You can then follow this link each time you have a scheduled session. You will need access to a computer, tablet, or smartphone with Chrome, Firefox, or Safari to join your session. Patients who do not have this capability may also contact us about telephone sessions, and we can discuss this possibility.
