After you have spoken with our office and set up your initial appointment, you will be emailed an invitation to set up your Patient Portal. Please follow the link to create your account, where you will be able to complete all Intake paperwork and sign required forms prior to your appointment. These MUST be completed at least 24 hours prior to your appointment. Please bring your license and insurance card to your appointment.
If we do not receive paperwork in advance, the Intake session may be canceled or rescheduled.
Please note that it is our policy to require consent for treatment from both custodial parents in cases of joint legal custody. This consent must be obtained prior to the first visit.
